Sometimes a license does not consist of all the information as requested in this application, in which case you are advised to leave the space in question open. Note that if your license is printed in another language other than in English you should be able to find much of your particulars printed in English within your passport or in your Government Identification Document.

I hereby authorize International Driver's Document Translation Centre © for the preparation of an unofficial translation guide of my valid national driving license. I do understand that this International Driver’s Document Translation Guide ™ is a supplementary document and simply acts as a translation of my valid foreign driver's license. It does not replace my license and it does not confer any legal rights or driving privileges. It must be accompanied by a valid government-issued driving permit. It is used when traveling outside of my home country.
I am not to present the International Driver’s Document Translation Guide ™ as the main document because it only serves as a translation of my National driving permit. Any mistakes or misrepresentation on this application will make the two documents non-matching and voids my attempt to use this translation guide.
I am not applying for this Translation Guide with the intent to use it as a substitute license or to be accompanied by an expired or non valid driving permit from any country. I further understand that this Translation Guide will become void if it is provided and then used incorrectly as described.
I further release International Driver's Document Translation Centre © from all liability caused by any misinterpretation or misrepresentation on my part. I additionally release International Driver's Document Translation Centre © from all liability caused by an applicant's failure to conform to any national, or international law. I further do verify that all the license information provided to be true and accurate.
